AIM: The aim of this study was to evaluate the clinical priorities and treatment of pneuomothorax due to subclavian catheter application. MATERIAL-METHODS: Thirteen cases of iatrogenic pneumothorax due to subclavian catheter application in Chest Surgery clinic between January 2005 and October 2007 were reviewed retrospectively. Catheters were applied to some of the patients before the operations and to the others at the clinics for various indications. For the treatment of the pneumothorax, expectant management, simple aspiration, chest tube drainage and surgical treatment were used. RESULTS: It is defined that tube thoracostomy is most prefered treatment intervention for pneumothorax. Whereas follow up and O2 treatment applied in two cases, surgical intervention needed in one case because air leakage continued for 10 days. Thorax tube was extracted after pleurodesis by talc in two cases whom negative ossilation duration was long. CONCLUSION: Consequently, iatrogenic pneumothorax as a cause of morbidity and mortality is a serious complication which may be treatable if it is noticed and measures are taken. In the catheterizations of thorax, it should be kept in mind that tension pneumothorax may develop when appropriate pressure conditions are formed even though pneumothorax is not clinically evident. AMAÇ: Subklavian kateter uygulanması sonrası gelişen pnömotoraksın klinik özelliklerini ve tedavisini değerlendirmek. GEREÇ-YÖNTEM: Ocak 2005-Ekim 2007 tarihleri arasında Göğüs Cerrahisi Kliniğimizde subklaviyan kateter uygulanmasına bağlı iyatrojenik pnömotoraks gelişen 13 olgu retrospektif olarak incelendi. Katater girişimi bir kısım olgulara operasyon öncesi dönemde, bir kısmına ile klinik ortamında ve çeşitli endikasyonlar nedeniyle uygulanmıştı. Pnömotoraksın tedavisinde gözlem, basit aspirasyon, göğüs tüpü ve cerrahi tedavi uygulanmıştır. BULGULAR: Pnömotoraksa yönelik tedavi girişimleri içerisinde en sık (%76.92) tercih edilen yöntemin tüp torakostomi olduğu belirlenmiştir. Pnömotoraks derecesi %20’nin altında olan 2 (%15.38) olguda gözlem ve O2 tedavisi uygulanırken, olgulardan sadece birisinde (%7.69)_ hava kaçağının 10 güne kadar uzaması nedeniyle cerrahi girişim gerekli olmuştur. Ossilasyonun negatişeşmesinin uzadığı 2(%15.38) olguda ise talk ile plöredezis sonrası göğüs tüpleri çıkarılmıştır. SONUÇ: Sonuç olarak, morbidite ve mortaliteye neden olabilen iyatrojenik pnömotoraks fark edilerek önlemler alındığında tedavi edilebilir ciddi bir komplikasyondur. Toraksa yönelik kateterizasyonlarda pnömotoraks kliniği gözlenmese bile uygun basınç şartları oluştuğunda tansiyon pnömotoraks gelişebileceği hatırda tutulmalıdır. Sonuç olarak, morbidite ve mortaliteye neden olabilen iyatrojenik pnömotoraks fark edilerek önlemler alındığında tedavi edilebilir ciddi bir komplikasyondur. Toraksa yönelik kateterizasyonlarda pnömotoraks kliniği gözlenmese bile uygun basınç şartları oluştuğunda tansiyon pnömotoraks gelişilebileceği hatırda tutulmalıdır.
