Misuse of Inhalation Devices-Effective Parameters and the Role of EducationYusuf AydemirKonya Numune Hospital
Aim: Inhaler devices used in the treatment of asthma and chronic obstructive pulmonary disease are often used incorrectly by patients. The aim of this study was to determine the rates of misuse of inhaler devices, to evaluate of the parameters that can affect incorrect use and to show the contribution of education to correct usage.
Materials and Methods: Three hundred patients, using inhaler devices, were included in the study. Age, sex, educational level, disease type and disease duration data of the patients were noted. Ability of the patients to use device before training were assessed according to the chart of patients’ ability to use inhaled medication which was prepared using the Turkish Thoracic Society asthma Management and Prevention Guideline. Face-to-face, hands-on training was then provided to the patients and their abilities to use inhaled medication were reassessed.
Results: 51% of patients used the devices correctly. The incorrect use ratio was 87% for pressurised metered dose inhalers and 47% in the dry powder inhaler group. Educational level, duration of disease, hospitalization frequency and pulmonologist diagnosis and device description were significant parameters influencing correct usage. Multivariate analysis revealed that state of education, gender, residence, duration of disease, pulmonologist follow-up, and hospitalization frequency were found to be significant factors affecting the proper use before education. The rate of proper use was significantly increased after inhaler device training (p<0.001)
Conclusion: The technique of inhaler device use must be taught to the patients. This education should be provided by the prescribing physician and must be repeated by a well-trained healthcare staff member. İnhaler Cihazların Hatalı Kullanımı-Etkili Faktörler ve Eğitimin RolüYusuf AydemirKonya Numune Hastanesi
Amaç: Astım ve kronik obstrüktif akciğer hastalığı tedavisinde kullanılan inhaler cihazlar, hastalar tarafından yaygın olarak yanlış kullanılmaktadır. Çalışmanın amacı; inhaler cihazların hatalı kullanım oranlarını belirlemek, yanlış kullanıma etkili olabilecek parametreleri değerlendirmek ve doğru kullanıma eğitimin katkısını göstermektir.
Gereç ve Yöntem: Halen inhaler cihaz kullanmakta olan 300 hasta çalışmaya alındı. Hastaların yaş, cinsiyet, öğrenim durumu, hastalık tipi ve süresi gibi bilgileri toplandı. Hastaların eğitim öncesi cihazları kullanma becerileri, Toraks Derneği Ulusal Astım Tanı ve Tedavi Rehberinden yararlanılarak hazırlanan inhaler ilaç kullanma beceri çizelgesine göre değerlendirildi. Ardından hastalara yüzyüze uygulamalı kullanım eğitimi verildi ve kullanım becerileri tekrar değerlendirildi.
Bulgular: Hastaların ancak %51’i ilaçlarını doğru şekilde kullanmaktaydı. Hata oranı ölçülü doz inhaler grubunda %87, kuru toz inhaler grubunda %47 idi. Öğrenim durumu, göğüs hastalıkları uzmanı teşhis ve tarifi, hastalık süresi ve yatış sıklığı doğru kullanıma etkili parametrelerdi. Multivariate analiz sonuçlarına göre eğitim öncesinde; öğrenim durumu, cinsiyet, yaşanılan yer, hastalık süresi, göğüs uzmanı takibi ve yatış sıklığı doğru kullanım üzerine etkili faktörler olarak bulundu. Hastaların inhaler cihaz eğitimi sonrası doğru kullanma oranı anlamlı derecede yükseldi (p<0.001).
Sonuç: İnhaler cihazların kullanım tekniği hastalara mutlaka öğretilmelidir. Bu eğitim özellikle ilacı yazan hekim tarafından yapılmalı, poliklinik ve servislerde yetişmiş sağlık personeli ile eğitimler sık sık tekrarlanmalıdır.
