Non-Hodgkin’s lymphomas belong to a group of lymphoproliferative malignencies that differ in behavior, treatment and prognostic patterns. Over the course of the disease, non-Hodgkin’s lymphomas may affect the thoracic structures – especially the mediastinum and the pulmonary parenchyma. Although endobronchial Hodgkin’s disease is not uncommon, endobronchial non-Hodgkin’s lymphoma is extremely rare. We report a case of endobronchial involvement of non-Hodgkin’s lymphoma and make a review of the literature.
A 53 years old man was consulted to our clinic complaining of dyspnea, cough, sputum expectoration and pyrexia for a month. In his history, we attained knowledge of taking 6 cycles of CHOP chemotherapy because of diffuse large B-cell non-Hodgkin lymphoma by explorative laparatomy 7 months ago. We observed dyspnea, tachypnea, orthopnea, cyanosis and clubbing at this case and also auscultation of left hemithorax revealed decreased breath sounds and bilateral expiratory rhonchi. Volume loss of left hemithorax, pleural effusion, central mass, LAP?, diffuse mediastinal LAP, right hilar LAP and multiple nodule formations of right hemithorax were observed by using thorax BT. Bronchoscopy revealed a pink-white coloured left main bronchus obstructed by vegetan, hemorrhagic mass lesion. In immunohystochemical exploration of bronchoscopic biopsy material, we found pancytoceratine and CK7 negative, CD45 positive, consequently diagnosis was reported as “diffuse large B-cell non-Hodgkin Lymphoma”. After two cycles of CEOP chemotherapy, dyspnea, pyrexia and cough occurred. The general situation of the case went bad step by step while taking the treatment for lung abscess + dyspnea and finally he died. We reported this case because of involvement of non-Hodgkin lymphoma acting like a lung cancer is rare and has an aggressive prognosis. Keywords: endobronchıal involvement, non-Hodgkın lymphoma
Non-Hodgkin lenfomanın endobronşiyal tutulumuAyperen Kunduracıoğlu, Aydan Çakan, Ayşe Özsöz, Nur Yücelİzmir Göğüs Hastalıkları Ve Cerrahisi Eğitim Ve Araştırma Hastanesi
Non-Hodgkin lenfomalar davranışı, prognozu ve tedavisiyle farklılık gösteren lenfoproliferatif malignensiler grubuna dahil edilir. Tüm hastalık sürecinde non-Hodgkin lenfomalar özellikle mediasten ve akciğer parankimi olmak üzere torasik yapıları etkileyebilirler. Her ne kadar endobronşiyal Hodgkin lenfomalar nadir olmasa da, endobronşiyal tutulum gösteren non-Hodgkin lenfomalar son derece nadirdir. Bir aydır nefes darlığı, öksürük, balgam çıkarma ve yüksek ateş şikâyetleri mevcut olan 53 yaşındaki erkek hasta hastanemize başvurdu. Özgeçmişinde 7 ay önce laparotomiyle diffüz büyük B hücreli non-Hodgkin lenfoma tanısıyla 6 siklus CHOP tedavisi aldığı öğrenildi. Dispneik, taşipneik, ortopneik, siyanotik olan, el ve ayaklarda çomak parmak saptanan olguda, sol hemitoraksta solunum seslerinde belirgin azalma ve bilateral yaygın ekspiratuvar ronküsler duyuldu. Toraks BT de sol hemitoraksta volüm kaybı, plevral effüzyon, santral kitle, LAP?, mediastende yaygın LAP, sağ hiler LAP ve sağ hemitoraksta mutiple parenkimal nodül formasyonları izlendi. Bronkoskopide sol ana bronşun pembe-beyaz renkte, dokunmakla kolay kanayan vejetan kitleyle tama yakın tıkalı olarak görüldü. Bronkoskopik biyopsi immünohistokimyasal incelemesinde pansitokeratin ve CK7 negatif, CD45 pozitif bulundu ve sonuç “diffüz büyük B hücreli lenfoma” şeklinde rapor edildi. Uygulanan iki kür CEOP kemoterapisi sonrası nefes darlığı, yüksek ateş, öksürük yakınmaları ortaya çıkan ve genel durumu ileri derecede bozulan olgu tedavi edilmekte iken eksitus oldu. Akciğer kanserini taklit eden endobronşiyal tutulumlu non-Hodgkin lenfomalı bu olgu hızlı seyri ve nadir görülmesi nedeni ile sunuldu. Anahtar Kelimeler: endobronşiyal tutulum, Non-Hodgkin lenfoma
